Community Partnership

Community Partners

CCHD relies on partnerships with organizations, businesses, health providers and service providers across the community to maximize resources and realize the shared goal of improving the health and well-being of all residents of Clinton County. Learn more about some of these partnerships below.


Clinton County School TABLE (Team Adirondack Breakfast & Lunch Educators)

Serving nutritious meals and snacks, providing nutrition education, and advocating for school nutrition policies and physical activity programs in our schools creates a crucial link to physical and intellectual fitness for our children. The School T.A.B.L.E. is a unique group in our county working together to "promote healthy eating patterns in our youth through education and the provision of nutritious meals in our cafeteria classrooms."

Members include representatives from the Clinton County Health Department and school food service directors.  All public and private schools representatives in Clinton County are invited and encouraged to participate in the group. Meetings are held monthly throughout the school year.


 The Clinton County Chest/Breastfeeding Coalition

Because human milk is the optimal food for infants, local professionals and residents have teamed up to strengthen and expand breast/chestfeeding initiation and duration rates within our community. The coalition’s goal is to ensure all parents have the information, guidance, and support needed to feed their baby human milk.

Current partners of the Clinton County Breastfeeding Coalition include: Clinton County Health Department, Behavioral Health Services North, Child Care Coordinating Council, The University of Vermont Health Network – Champlain Valley Physicians Hospital, Plattsburgh Primary Care Health Partners, Mountain View Pediatrics, State University of New York – Plattsburgh, La Leche League of Plattsburgh, and several community members. Clinton County Health Department (CCHD) has partnered with local pediatric care practices to follow the feeding status of Clinton County’s youngest residents. Click here to learn more about infant feeding in Clinton County.


Action for Health Consortium

The Action for Health (AFH) Consortium serves as a sounding board and guiding entity for comprehensive health assessment and health improvement planning in the local community. Members play a critical role in the selection of health priority areas and associated strategies, sharing knowledge of and advocating for the needs of the populations they serve. Meeting about six times each year, the group then oversees the implementation of the local Community Health Improvement Plan (CHIP) and discusses obstacles, changing conditions and other circumstances impacting progress as it monitors the CHIP’s implementation.   

Current members include: Alliance for Positive Health, Senior Citizens Council, St. Joseph’s Outreach Center, Champlain Valley Family Center, North Country Healthy Heart Network, Town of Plattsburgh, Clinton County Office for the Aging, UVMHN-CVPH, Town of Peru, Center for Neurobehavioral Health – SUNY Plattsburgh, Cornell Cooperative Extension, Joint Council for Economic Opportunity, Clinton County Mental Health and Addiction Services, The Foundation of CVPH, Behavioral Health Services North, Plattsburgh Interfaith Food Council, Adirondack Health Institute, Friends of the North Country, Clinton County Youth Bureau, Clinton County Planning Department, and independent citizens.
